There's a form to fill out.
You also have to promise not to use as a daily commuting car. You are restricted to the number of miles driven each year.

I agree that you will need to go to the DMV but I have to respectfully disagree with the previous URL given for the MVC. The NJ MVC's official web site is http://www.state.nj.us/mvc/index.htm.
