if you lose more than 1-2lbs a week you are not losing fat - you are losing water weight and muscle - so needless to say - it IS comming back
find calorie/fitness calculator on the net, enter all your stats and it will tell you how many calories you need a day to maintain, take off 300-400cal off that number and thats how much you need to lose.
eat 5-6 small meals a day (eat every 2-3 hours)
5-8 servings of fruit and veggies a day
8 glasses of water
have complex carbs for breakfast - they give you energy
have lean meat (protein) for dinner - repairs muscle
cardio exercise 4-6 times a week for 30-50min, light weight training
dont consume foods that are made of white flour (white bread, cakes, past etc.), sugar loaded foods (cookies, icecream, candy etc) and nothing fried, oily.
ofcourse you can spoil yourself once in a while with a little treat:)

Losing weight "extremely fast" and not gaining it back is impossible. Unless you are willing to make lifestyle change, any weight you do lose on superficial, fad diets, such as atkins, will come right back on. Why? Because you can't live a life not eating carbs or drinking only lemonade, so eventually you wll have to eat normally again, and everything will come back. On top of that, if you restrict yourself that much, you are likely to crack and binge.
If you want to lose weight, you need to exercise (cardio 4-5 times a week for at least 40 min- 60 min each time) and do some weight training. It takes 3500 calories to burn 1 pound in 1 week. So as long as you consume less than you burn, you will lose weight. Losing 1-3 lbs a week (depending on your weight and how much you need to lose) is healthy. You will break bad habits and build healthy ones along the way.

You probably don't want to hear this, but you can't.
In order to lose one pound, you need to burn 3500 calories. A lot of fad diets and programs promise 10 lbs in 10 days or whatever. It's nearly impossible to burn 3500 calories a day, especially when the body only really needs around 2000 (depending on each person). The fastest you can lose fat is a rate of 1-2 pounds a week. Everything else is water weight and/or muscle.
If your just looking to lose weight, then just about anything will work, however, since the weight comes from water and/or muscle, you'll gain it all back very quickly.
If you want to lose fat, then you need to find a good solid lifestyle change (not diet). Try looking into the glycemic index (http://www.glycemicindex.com/). Also try talking to your doc (however, even though docs are smart, they don't always know everything, especially when it comes to fat loss).
I also recommend hypnotherapy. Hypnosis works wonders. Try finding a hypnotherapist near you. If all else fails, you can buy recordings from the Internet such as http://www.stevegjones.com . If you use a recording, I recommend learning about hypnosis first.

The best way to lose weight really fast and keep it off is to cut your leg off at the knee. If that doesn't appeal to you then you will need to work hard and eat right. Eat 5 to 6 times a day, burn more calories than you take in and eat healthy. Stay away from fast foods. You need to do weight training as well as cardio. Keep this up and you will see dramatic results shortly. The only "extremely fast fix" is a hacksaw.

WATER. Drink A LOT of water. This will flash out all the poison and fat out of your system. Do intense cardio work out in the morning with empty stomach at least for 10 min to 30 min 3 days a week. Your body will use stored fat for energy this way. If you want to keep body fat to come in, you must do weight training. The more muscles you have, the less space for the fat to come in.

pick up a 5 or 10 pound weight at the gym and visualize that weight coming off holding the weight in your hands helps bring home just how heavy even 5 pounds of extra fat can be

Any healthy weight loss requires time and retraining of your eating methods. I suggest L.A. Weight loss Centers. If you have one near you, check it out! My mother-in-law lost over 100 pounds and went from a size 16 to a size 6. She looks great! They teach you to eat right and change your lifestyle. I think she did it in about a year and a half.
