No. They must give them to you as soon as you ask for them and photocopies of medical records should never be taken unless the patient (you) are photocopying them, so they are not entitled to keep a copy for themselves either.

Do you recall signing anything that stated the original copies of medical records must stay with the doctor? If you did, then you probably won't be able to. Why don't you just have the doctor send YOU the copies and you can give them to the new doctor. So long as you are of age and sign a statement attesting they are YOUR records, you won't have a problem.

They can charge you for the copies if they want to. Usually it's per page. You will have to sign a release, you can't request it on the phone.
I personally have never been charged, and I get copies of all of my medical records. If you have another doctor send a signed release from you, they have to provide the doctor with that information.
They can NOT deny access to your medical records if you still owe them an outstanding bill.

No, you usually tell your new doctor who your old doctor was, and the old doctor faxes all your records to the new one. If you want to get them yourself, then they may charge you per page for the copies. You will have to ask for copies because they are not going to give you the originals. It really isn't some conspiracy, its just as much as kinkos would charge you to make copies. Like I said though, they will do it for free if another doctor calls them and requests that they be sent. I have had to do this many times. If you do want copies for yourself though, I suggest you call them ahead of time and tell them that you are coming in (especially if your file is big). Sometimes they are too busy to make copies of your entire file, and calling them will give them time to have it ready for you.

Yes, you usually have to pay. There is a schedule of cents per page you must pay. The Dr. has to make a record of providing you the chart and sometimes copying all or part of it in case you write him to have some of the info transferred to another Dr. In some cases ther is a law requiring him to keep a copy.

it depends on the doctor..if you want the records shipped to your new doctor directly from your old doctor, they may charge..but if your picking them up, they no they should not charge
