I am sorry for the loss of your grandfather.
This could be a difficult situation, depending on the feelings of your relatives.
It might be appropriate to have a private prayer with your grandmother if others there are offended by your Christianity. On the other hand, they might welcome it. In this situation, follow your feelings. Your heart will know what to do, though you may not know until the moment comes.
Be respectful of others' grief. Everyone's grief is personal. Be willing to listen more than talk. Especially be willing to talk about your grandfather. Laughter helps a lot, in small doses.
I hope it goes well for you!

A prayer is ALWAYS appropriate. It can never be wrong to have a talk with our Creator. It would be fine to thank God for the time you had with your grandfather, for the love he showed you, for the ways his life influenced you to be a better person, and then ask God to be gracious to your grandfather and keep him in his care. What a nice thing it is that your family is going to visit his grave together.

The moment is important -- prayer can come like a hammer or a pillow. You want it to "be there."
If it is, you might say something like, "You all mind if I offer a short prayer?" I have yet to hear someone decline that.
Then something like, "Dear God, I want to thank you for Grandpa, thankyou for the husband he was to grandma, and for the gift he was to our family.... (etc.)
I also thank you for helping us as a family to get through the past year, especially Grandma. Through the pain and loneliness she has relied on you and we thank you that you have not let her down.
We remember Grandpa today along with the great memories, and we look to you to further guide our lives as we follow Jesus. Amen."

I agree with the other answers. Actions speak louder than words.

If I were you, I'll just bring some tools along to trim the grass and clean the place up, put up some flowers. No need to pray.
