Sometimes you do. Sometimes you don't. It depends on the injury.
Many people sadly focus on physical problems (such as reduced control of limbs) but deny any changes in their ability to process information or make decisions (what are called 'executive functions').
So while a medical diagnosis may tell you you have been severely affected by your injury you may simply not be able to 'see' it I'm afraid. Which makes life difficult for everyone.

Well, you get in a trauma like a car accident or boxing match where your head gets hit really hard and then you go to the hospital where doctors do a cat-scan of your head and they tell you.
