From a culinary point-of-view, it simply means to mix up the ingredients in a salad. Traditionally, chefs have done this by carefully tossing the ingredients in the air above the bowl, hence the "toss" part, but ultimately that is not necessary. That said, the phrase has since been used as various means of slang, as is evident from some of the answers here!

If you add layers of vegetables to your salad, it is when you take fork and spoon (or tongs) and gently lift and mix everything together. It could also mean when you add salad dressing to the whole bowl (instread of your own individual salad) and do the same thing. You "toss" instead of "stir," but hopefully your tosses won't land lettuce leaves on the wall like they did mine recently.

Put salad in a big bowl, using 2 wooden spoons or your (clean!) hands, scoop up from the bottom and plop on top. Repeat untill salad is evenly mixed.

in my opinion it means putting all salad ingrediants in a bowl including salad dressing and mixing thoroughly..i wouldn't know about the slang way i'm older 42,

Well, it's gross, but it basically means tonguing someone in the anus. Maybe, according to the Chris Rock prison comedy routine, using honey or syrup to disguise the probably disgusting taste.
