To perform SHILLINGS test you need:

injectable B12
an oral dose of radiolabelled B12
urine sample collection materials
a method to detect the radiolabelled B12
intrinsic factor

You give the patient a dose of B12 by injection
You then give them a dose of radiolabelled B12
You collect their urine and test for the presence of the radiolabelled B12

If there is less than 5% of the oral B12 dose found in the urine, repeat the test with intrinsic factor. The amount of B12 found in the urine should be much higher proving that the B12 deficiency is due to lack of intrinsic factor.
