You are correct. Distractions lead to surprises, which become accidents. Distractions account for about 43% of all accidents.
Distractions include things such as cell phones, radios, food, passengers and external distractions. Passengers are the number one cause of distracted accidents.
While drinking and driving accounts for a higher percentage of fatal accidents, drinking is only a factor in 25% of all accidents. These accidents are more avoidable, and more than half are one car accidents.

Distracted Drivers
Mark Edwards, Director of Traffic Safety at the American Automobile Association stated, "The research tells us that somewhere between 25-50 percent of all motor vehicle crashes in this country really have driver distraction as their root cause."
The distractions are many, but according to a study conducted by the Virginia Department of Motor Vehicles (DMV) and Virginia Commonwealth University (VCU), rubbernecking -- or slowing down to gawk at another accident -- caused the most accidents, accounting for 16 percent of all distraction-related crashes.

It's a combination of things not just one. However, the combination has a common thread and it is distraction. The other additives are:
1) not fully understanding the capabilities of your car and the limitations of your car. Just because you have 4 wheel or all wheel drive doesn't mean you can drive faster in the snow and rain. Riding the tail of a Honda Accord is not a good idea if one is in a truck the stopping radius of the truck will always be greater.
2) assumptions- just because your light turned green doesn't mean people won't run the red.
3) looking to make sure no one is in your blind spot. Be honest how many times have you been on the freeway or just a simple 4 lane road and watch near misses because the people didn't bother to turn their head to see if someone is in their blind spot. Which leads me to #4
4) not bothering to look in one's mirrors at all
5)inconsideration just because one may see a blinker a mile up the road does not mean one should speed up and prevent them from getting in front.
A good portion of what we learn in Driver's Ed. we tend not to do once we have been driving a couple of years.

Well, driver error anyway. Speeding or driving faster than is safe for the conditions is surely a common contributor to accidents.

That's right. The reason for most accidents is driver distraction (using cell phones) and secondly is drunk driving.
