You need to check with your state. Generally, you do, but not always. You do for certain if you have a storefront.

Of course No. I mean depends on how many cookies you're going to sell to your clients, if you are planning to sale it in nations wide, may be you need one other than that i think' you don't need it'.

It sort of depends. Are you doing it as a "hobby", or to be a real business and market yourself? My aunt had a professional bakery in her basement, but all her clients were just friends, relatives, and friends of friends of friends. But she never advertised as a business, so she didn't need to have the health inspector in.

Check with your local Board of Health. I believe they do inspections of baking facilities but how they know where to look is unknown. They could probably answer your question.
