Notify your friends and family of your noble intentions!
Download, print, and carry an organ donor card.
Make sure it is signed, and you either check the "Donate any needed organs or tissues" box, or you write in "Eyes" if you checked the "Only the following..." box, according to your wishes.
Take the appropriate steps to have your driver's license reflect your desire to donate.
Make sure your desire to donate is reflected in any living will or advanced directives legal document you make.
Consider visiting a local organ donation organization, and discussing with them what you can or should do to make sure your wishes are fulfilled.
Understand that you will be examined post-mortem for whether your organs and tissues qualify (considering your cause of death and the health of your organs/tissue). You should make it known while you are living, however, if you have any conditions or diseases which would make you an unsafe or unviable donor.
Rest a little easier knowing that even if the worst had come to pass, your charity and kindness will live on, and may even restore to someone the gift of sight

Why forget the great Yogi 'Shiva'. He got the head of Elephant transplanted on Ganesh. He drank the poison and became 'Nilkant'. He took the flow of Ganga on his head. GOD can do all these when good people are in trouble and He has a mistake. ...... but .... What ever GOD has decided for bad ones .... faulty body due to his 'Karma' must not be changed. Even Saints suffered by helping such cases. It is like helping a thief get out of prison, which the judge has pronounced ! Remember there is big business in these donation and some poor also sometimes suffer. Positive is poor earn by some loss ! Blood donation is a big political service now-a-days.

When you get your driver's license, you can enroll in the donor program and specify which parts you want to donate. If you don't have a d/l, you can still enroll in the program with a state ID card. Para medics, firefighters. police officers and other first responders are trained to look for these on victim fatalities. Right now, I'm still using my body parts.

Get yourself a donor card (usually on back of your driver's license or photo ID). There are spaces you can fill out instructing what can be donated. Have it witnessed by 2 people and then make sure your family and dr know your wishes.

If you are in Britain or Australia it is also on your driver's license, the other thing you can do it tell your next of kin, they are the only people that can give permission after you die. Putting it in your will would be no good because by the time your solicitor is contacted and it is read it will be too late.

You might be able to list your preference on your driver's license.
In New York State there is a box on the back of driver's licenses to check, and even list which parts you are willing to donate. (This is also true for DMV issued Non-Driver ID's)
Check with your local department of motor vehicles, or other licensing agency.

Look up organ donor registration on the net, and they will give you the address of the office nearest you.
I think local hospitals have info on that.
Then you sign a donor card and carry it with you...
just in case.
