The usual things noted are:
color (good, pale, waxy, flushed, etc)
condition (warm, dry, cool, clammy, etc)
birthmarks
scars (from injuries, prior surgeries)
rashes or lesions
acne
anything abnormal: signs of redness, bruising, infection, profuse sweating, swelling
The nails are part of the integumentary system also so be sure to note anything unusual about the nails as well.
Be sure not to diagnose in your description (unless you are studying to be a doctor). For example, don't say "fungal infection noted in left large toenail". Instead, you would say, gross discoloration noted in left large toenail.
