I work from home for a psychiatrist and am totally confused with is new NPI stuff! Where exactly does the NPI # need to go on the hcfa? And am I required to add the taxonomy # as well if it's mental health? Is anyone using the new forms already, since it's not required until June now?

the address says it is the new version but the title says it is 8/05. there is the split NPI field discribed which is the major change.
