Where are you planning to settle down after medical school? Depending where you will be moving too, check that area out and see what kind of population you have.... You don't want to take up Korean and work in a hospital situated in an Haitian neighborhood! If you have the time, dedication and the willingness to submerge yourself in another culture you can learn American Sign Language... Even if you don't become fluent it could be helpful later on when you come across a deaf or hard of hearing person. It is very difficult for the deaf community to communicate and there is a shortage of ASL interpreters in the US and even when the interpreters are available many times a lot of meaning gets lost in the translation.... Think about it, you will enjoy it.

I believe Mandarin Chinese is spoken by more people in the world than any other language so that seems like a good option.
I'd pick something quite different. You have two romance languages down so you can really figure your way through the others (and folks from western Europe speak multiple languages so you should be able to communicate).
Something like Russian or Chinese or Farsi would give you an incredible variation that should prove invaluable to the medical care you will be able to provide.

Latin would be best. There is a lot of terminology you can pick up easily in med school if you learn Latin.
Not to mention French and Spanish are derived from Latin, so Latin will sound a little familiar. However, Latin is harder than French and Spanish.

Romanian - really eazy since u already know french and spanish, very similar to Italian, which u should also learn and the most similar to Latin from all the Romance languges

I would choose Latin. Many of the terms you come across will be derived from Latin.
