Many pastors simply say" I baptize you in the name of the Father, the Son and the Holy Spirit".

In the New Testament there is no case of a prayer being said or of any standard formula of words being pronounced. The essential thing is the understanding of the person being baptized regarding the meaning of baptism and the commitment being made. Baptism is something done from the heart (Romans 6:17) and a personal pledge to live for God and to give up the practice of sin (I Peter 3:21). Romans chapter 6 gives the full meaning of baptism. The words Father, Son, and Holy Spirit mean that the one baptized understands that they are entering into a personal relationship with God and becoming a member of His family and obedient to his teaching. This why some places in scripture say only baptized in the name of the Lord or in the name of Jesus Christ.

Whatever prayer is said, should come from the heart of the believer.

Jesus commanded the apostles to baptize in the name of the Father, Son, and Holy Spirit.

I baptize you in the name of the Father and the Son and the Holy Spirit is the formula to use, however lay people should only baptize someone in danger of death otherwise it is best that a Priest or Minister do this.

Matthew 28:19 baptizing them in the name of the Father, and of the Son, and of the Holy Ghost:

The prayer should at least have this part in the baptism.

You should only be baptized by a clergyman except when in extreme danger of immanent death. It is not to be taken lightly. Use the person's name and say, "N., I baptize you in the name of the Father and of the Son and of the Holy Spirit".
