I believe you are holding a Saudi Arabian Civil ID. I love to help you, but I can't guess anything from the hints that you have given.
Edit:
In Kuwait ID, the details should be in below mentioned order:
Name:
Nationality:
Date of Birth:
Gender:
Date of Issue (the card):
And the rear side the details of sponsor and residence address. (No religion column)
