There are a number of reasons for this cachectic = wasting situation.
The main ones are that the cancer tends to decrease the appetite, as do many of the drugs and the radiotherapy used to treat it. The cancer may also use up a lot of the calories, in the same way as occurs in a pregnancy.

probably because cancer cells use the energy for themselves, and they have faster metabolism, so cancer grows
