If the account of Genesis is accurate (which I doubt) God created Adam and Eve, and nobody else.
Therefore, it is safe to assume that Cain most likely married his sister.
Remember that incest was an acceptable practice in most ancient cultures, even in the Greek and Roman Empires. The practice continued to the early part of the middle ages. But during the Renaissance incest was no longer acceptable.

Genesis 4:17 says, "Cain lay with his wife, and she became pregnant and gave birth to Enoch." Who was his wife? It is implied in the biblical text that Cain married one of his sisters. Several facts lead us to this conclusion.
First, it is clear that Adam and Eve had a number of children. Genesis 5:4 says, "After Seth was born, Adam lived 800 years and had other sons and daughters." Since Adam and Eve were the first man and woman, and since God had commanded them (and their descendants) to be fruitful and multiply (Genesis 1:28), it seems reasonable to conclude that Cain married one of his many sisters. It is also possible that he married a niece or even a grandniece.
One must keep in mind that in the early years of the human race there were no genetic defects that had yet developed as a result of the fall of man. By the time of Abraham, God had not yet declared this kind of marriage to be contrary to His will (see Genesis 20:12). Laws governing incest apparently did not become enacted until the time of Moses (Lev 18:7-17; 20:11,12,14,17,20,21). Hence, there was no prohibition regarding marrying a sister (or niece or grandniece) in the days of Cain.

Adam and Eve had sons and daughters. (Gen. 5:4) Cain would have married one of his sisters. Now that sounds gross to us - but that is because, today if you marry a close relative there is a greater chance to pass on a common genetic mistake (unmasked) to the child. However at the time right after the fall there were no genetic mistakes and no fear of pass on genetic errors.
Later on in history the book of Leviticus forbids the marrying of brothers and sisters.

His twin sister, Lulawa, according to one of the "Lost Books" of the Bible.
However, some believe there were primitive people already living on earth when Adam and Eve arrived, so that Cain married one of these aboriginals. One fellow told me that he married a green-skinned Lemurian woman! Another fellow thought he married a monkey!! Some of the pre-humans may have been somewhat monkey-like in appearance, small and covered with hair. For that matter, some believe Adam and Eve belonged to these homid types, rather than a more advanced race from space. Another version still tells us that Eve was a stranded space traveler and that she took a Neanderthal man (adam) for her mate.
Take your pick, or make up your own version.
