The reason your diets are failing is because they are calorie restriction diets. If you say that you must stick to a certain calorie count and not eat any sweets or cookies or fat or anything that tastes good, you will end up cheating. And with cheating comes the guilt, and the end of your diet program.
Instead, try to follow a good eating program. Try to keep your calories at a certain level, but give yourself 200-500 calories lee-way either way. Also, don't swear that you won't have ice cream, just try to have a smaller portion. And if you know that you will have too much ice cream if you buy it, then don't buy it at all (try healthier options like frozen yogurt). Just try to eat healthy and follow the food pyramid plan.
Eating fruits and vegetables is the easiest way to cut calories because they are naturally low in calories and they contain fiber, so you will fill up faster. A professor once made the comparion between ice cream and apples. He said that for the same amount of calories you can have one cup of icecream or something like 15 apples. And think about how full you feel after one cup of icecream... now think about how full you feel after eating just one apple.
Good luck with your "eating plan" :)
(Oh and by the way, even if you are on this healthy eating plan and you do go crazy one night and eat like 5 cups of icecream, make up for it the next day by exercising and DON'T BEAT YOUR SELF UP. Everyone has off days and everyone deserves a treat now and again.

The average person weighs more six months after a diet is finished.
Two ways to stay on your diet:
1. Eat right and exercise. Make changes that you can hang on to for LIFE, not a couple of months.
Or, if you're happy with just short-term weight loss,
2. Buy a really cool clothing item one size less, and aim to fit into it in two months.
But thimg about 1.) :-) You'll not only look better, you will feel better too, and live longer.

The real answer IS to eat right and exercise. That's the only way to lose weight healthily.
Dieting is a temporary fix, and you'll end up putting on all the weight after you stop, and probably more.

find calorie/fitness calculator on the net, enter all your stats and it will tell you how many calories you need a day to maintain, take off 300-400cal off that number and thats how much you need to lose.
eat 5-6 small meals a day (eat every 2-3 hours)
5-8 servings of fruit and veggies a day
8 glasses of water
have complex carbs for breakfast - they give you energy
have lean meat (protein) for dinner - repairs muscle
cardio exercise 4-6 times a week for 30-50min, light weight training
dont consume foods that are made of white flour (white bread, cakes, past etc.), sugar loaded foods (cookies, icecream, candy etc) and nothing fried, oily.
ofcourse you can spoil yourself once in a while with a little treat:)

what do you expect to have real answers? some people who ans here are real experiences and some are the specialist. believe it or not, its all up to you. to stay on balanced diet, lots of way to do that, but it all up to you to believe, its all about eat right and exercise right. if you dont either eat right or exercise right, its pointless as you either will look skinny or very lean without much good looks. if you cheat once a while, its still ok but not all the way.
this is my real ans and experience.
3 meals a day, breakfast is a must. whereby breakfast eat at low quantity but high quality. for eg, take cereal or oat meal. if wanna take bread, pls take the wheat bread which is healthier and better. the most you take for bread is only 2 slices, depend on your calory consumsption. as for drink, dont take a glass full of hot chocolate, but i suggest a glass of low fat milk or cereal drink. cut down sugar and sweetened milk cos these have high calories.
as for lunch, you can try to take high quantity, but balanced quality. mean tat balanced meal with 1 portion of meat, 1 portion of vege, i portion of egg or tofu. try to cut down rice, maybe half a bowl. if don feel like eating rice then you to take noodles soup or mushroom soup with salads.
as for dinner, low quality and low quantity food, more on fiber and protein food but less in carbohydrate food. tats mean cut down rice and meat, if possible. if can, pls eat some fruits, like banana, papaya or apple before the meal at least half an hour before meal.
remember, each meal must not be taken too full. about 70% full is enough. all it takes is discipline, you can have strong desire to slim down, i believe you can do it. oh yes, don you ever take SUPPER. If you sleep after 12am and begin to feel hungry badly, take a glass of milk can do or worst a glass of cereal drink and hot chocolate. dont consume additional food like biscuits and bread, PLSSSSS!!!!!
im doing the same too, doing my very best. in between, you can do exercise to sweat a bit. i do light exercise at least 15 minutes everyday, but weight lifting about 3 times a week. light exercise mean stretching, sit up, belly dancing or skipping, something easier to do or do house chores then. heavy exercise is like playing basketball, weight lifting jogging and swimming if you have such hobbies.
always feel positive when doing all this as these do affect the result. always feel happy about yourself and have self confidence that you can cut down your weight.
try it out for 1 months and i believe you can see the result. you may not see the body shape change so much if you dont do lots of exercise but you personnaly would feel lighter and not easy to get tired. along the way if you have gastric problem or other symptomes, pls drop in to let me know. i used to have great interest in nutirtion and maybe i can recommend you sth else if you have other issues.
so, good luck
