I guess it depends on why you were invited. If it's your best friend in the whole world, but it's really a family gathering, she may have invited you to be polite and not necessarily want or expect you there. In which case, say that you feel that you'd be intruding on family time and ask if you can go out with her some other time to celebrate.
If it's a family member and the party is more of an age/peer group/gender appropriate party that you won't fit into, again, ask if you can take her out later.
If it's someone you're not at all close to and can't imagine why you were invited, any of the excuses given by others should be fine. Keep the excuse simple and don't involve people that both of you have contact with. Avoid anything that sounds lame like "my grandmother got sick" or "I had to babysit", as those will generate more hostility than just going to a party that you'll hate.
If she's a good friend and none of the above apply, you should try to go, even if you know (or suspect) it's going to be a bad party, as she will need morale support and a friendly face or quick laugh will help her on her special day.

Your sick,you already have plans,you have plans with a family member and you can't get out of it, you have an emergency,the truth ( you just don't feel like going),you're going to be out of town,you have to baby sit, you have to go some where,you have a migraine,you have an upset stomach and you have the runs,you have a fever. I think you should go with the truth though. Good luck PS. I have an inlaw that never accepts the truth,she tries to argue when you tell them how you feel about something or that you just don't want to do something. The truth is never good enough for her. I can't stand that. Try and stick to the truth though.

Something came up>
Had to change the air in the spare tire.
My dog died and I needed counselling.
Mom came by for a visit as I was walking out the door.
Test tomorrow and I had to study.
Got a flat on the way over by the time I got it fixed it was to late.
Dont do parties I hate to have fun.
My religion doesnt permit the celebration of birthdays
Mom said no.
Worn out from work sat down before getting dressed and fell asleep.

Ok do you mean reason? a good reason not to go, is because you just don't want to go, or you don't like them. An excuse not to go, is you have a headache,bird flu, cramps. your best bet, is to tell the truth, like" I really don't get along with Ms X you invited, so not to cause i commotion, i will not be attending, but know I like you. " because the people always find out at some point in time, then you will look like a fool.

Are you looking for an excuse...call the morning off and say " I have been up sick all night and have the flu. I am afraid I would be spending th entire night in the bathroom if I come. Have a happy birthday and I'm sorry I missed it."
