Hot dish here in Pennsylvania basically just means a hot meal. We(or at least I and people I know) say casserole for a combination of ingredients baked together.

We say it in Maine. I work in a kitchen so, I hear it all the time. Also people say it when a good looking woman or girl walks past ya.

'Hot dish' in the midwest, MI, IN, IL, usually means a reference to a pot luck entree that requires a crock pot. for example:
"I will be bringing a hot dish, I need to be close to an outlet."
Casseroles are full of carbs and fat, I think I've made one in my whole life and think they are gross and mushy.
