Well, the username would be admin and your pass would be admin. I recommend not messing with the settings in the page that would open though...

You would have to check your exact router's documentation, but there should be a 'hard reset' option provided. It's a way (usually on the router itself) to reset it back to default.
On my DLink, there's a small reset button on the back next to the lan ports. You could check there first and see... if that doesn't work check the manufacturer's website to find out how to do a factory reset.

Thats your routers password, read the instructions, different model uses different settings.

User name: admin
Password: password

Works on my Westell router, anyway.
