It is used for extraordinary situations. In fact, the person who asks this question understands her/him. But that person may not want to believe the event.
For example:
Jessica: I don't wanna see you anymore.
George: Why? What do you mean?
Jessica: I mean that our relationship doesn't work.
Georger: What! What do you mean?
Jessica: I mean that I wanna kill you now.
