Almost all trains are equipped with lavatories. If they weren't there'd be an awful lot of cleaning for the crews.

Depends on the train.

Intercity trains (Amtrak and Via) - all have restrooms.

Commuter trains (suburb to city) - some do, some don't. Newer systems tend to have restrooms, especially those which use the "Go Transit" octagon shaped car http://www.railpictures.net/viewphoto.php?id=88615

Subway trains - None have restrooms. But some stations do.

City streetcars - None have restrooms.

Every train I've been on has had, at least, one restroom at the end of each car. Check with the company that operates the train that interests you and look under "fleet" or "our trains".

Amtrac does. A ittle tip.. The trains I use run from Washington to Boston.
When I get on in Phila, they're still fairly clean, by New Haven, not so clean, by Providence,not clean at all. Plan accordingly.

All Amtrak trains have restrooms on them. Not sure about other passenger trains.

All of our freight locomotives have restrooms in on them.

It's a lot harder to find them on local "commuter" lines but even then I believe there are some cars equipped with them, but definitely not all of them! You might have to walk a car or two back. Just beware! A lot of times they aren't the most clean!!!

I know that the Septa Regional trains in Philadelphia don't have any bathrooms

Yes. They are usually in the control car on commuter railroad and can accommodate wheel chars. Amtrak has them as well.

I WORKED FOR A COMMUTER RAIL SERVICE AND THE CARS WERE TRI LEVEL CARS MADE BY BOMBARDIER OUT OF CANADA AND THERE IS ONE RESTROOM PER CAR

if they don't that would be scary, cause even short distance trains like metro-link have bathrooms in them.
