You haven't officially handed your notice in until it is in writing. If you don't want to burn your bridges, always a good thing then write something along the lines of -

as per contract I wish to give you "x" number of days notice of my intention of resigning from employment with "company name". This is in no way a reflection of the company or the colleagues i have worked with in the past "x" of years.

It is always better to leave on good terms as you never know when it will be a cold day at the soup kitchen and they are the only alternative you have left.

you put the date you want the notice to start and for how long and the reason you want to leave. For example.
dear sir or madam
re: notice to leave
from 11/5/07 I want to give one week notice because I have been offered another job.

I would write that you are formally tendering your resignation and put in your official leaving date. To leave on good terms, I would also put in some blurb about wishing the company all the best for the future and saying that you have enjoyed your time with them. Enjoy your new job!

Thank them for their employment and the opportunity to learn more. Also specify your last day with them. Remember to never burn a bridge. It seems that the world gets smaller and smaller and you may need that bridge at some point in the future.

Just make it simple, straight to the point and professional. Be sure you are on good terms with them so you can use them as a reference.

you want to resign from the date you specify no other information is required because by rights you should have a leaving interview where you can air your greviances if you have any
