Any suggestions for the best replacement for Lotus Organizer?

I use it now to keep my appointments and To Dos and I like it's layout like a Filofax book but it's interface is not sufficiently customizable.
I already know about Google and Yahoo calendar but want a program that sits on my hard drive.
