are we talking about a daughter or a g/f??
daughter - get and keep open communication with her. explain all risks invovled and be prepared to back it up with written proof if needed (websites can help explain risks as in std's and you may even find something about young pregnancies). sometimes kids shut parents out with the attitude - you just think you know it all... and they like what their classmates/peers tell them better cause at that age - kids know everything and parents know nothing. kids are invincible - so they think till it happens to them.
g/f or wife - if you need a chastity belt to keep her faithful - your relationship is already done. call it quits before she brings something home to you that you don't want or before she straps you with a baby that may or may not be yours.
g/f or wife - not cheating but a higher sex drive for you than what you want - enjoy it while it lasts. women's drive can vary greatly at times and if her drive dies and yours picks up - trust me, she'll remember all the shut down's you gave her.

Only if both of you want one. If you have doubts about one's faithfulness, I don't think a belt is going to solve those issues.

No you should learn to trust her. Locking her in chains is going to do no good, but trusting her will mean a whole lot more and give her more of a reason to love you, not hate you!
