Some of those puppets were made by kids as young as your son. They used a pattern they got on Project Puppet dot com.
If I were to use a muppet foam pattern from that website for a frog--the Roly one looks best to me.

You could use pieces of green felt (bought at local walmart store or craft store). Cut out a frog like shape (2) and glue/contact cement them together. You could give him dangling legs made out of the felt. Give him a long toungue maybe put a fly on the end of it (black dot out of felt with 2 white felt wings). Cut out some eyes....could use black and white felt as well for that. Since the felt is so cheap project would probably cost well under 5 dollars.

You can get a styrofoam circle, green paint, black felt and green felt all from Dollarama. First you paint the circle green cut black circle for the eyes cut a line across the circle for the mouth and glue red felt that is shaped like a tongue and then you glue the green felt around the head or you can buy a doll dress and glue it around the head and you can say that it is a frog from a fairytale. All the stuff you can buy at dollarama or any dollar store.

If you really can`t make one after all the suggestions - just buy a stuffed soft toy frog, cut it open at the bottom, pull out the stuffing, and stick it on his hand - that`s not hard - even i could do that - i think...........................

if you cant sew, you should either try using a green sock or some felt with superglue. added to this should be some googly eyes and some red felt for a mouth, also other froglike features.
