You might want to check out at least this page on claying with disabilities at my polymer clay encyclopedia website:
http://glassattic.com/polymer/disabilities.htm
It has a lot of suggestions for various types of disabilities, plus ideas for tools, etc., that can help, as well as links to other sites with ideas.
You might also want to look on this page for some additional ideas that are easier to do in general within various categories of polymer clay:
http://glassattic.com/polymer/kids_beginners.htm
And if there's any particular area of polymer clay you're especially interested in, you should also check out the Table of Contents page for the whole site:
http://glassattic.com/polymer/contents.htm
...then scroll down to browse all the pages, categories and subcategories. When you find one you want to check out, click on its name from inside the alphabetical navigation bar on the left side of the page to go to it. There should be lessons for various things on each page that you could do depending upon the disability.
Also, if you don't already know about them, there are polymer clay groups online you might really enjoy... there's even one called clayingwithdisabilities. For info about all those groups, look on this page:

Making a collage seems like it wouldnt be too hard and would be great for expressing yourself creatively. If scissors were too difficult you could even rip pictures or images out of magazines etc. to use as well as letters etc. Painting is also an option. There was this really incredible and inspiring movie I saw when I was a kid about this women who was paralyzed from the neck down and she held a brush in her mouth to paint these beautiful and amazing paintings. Her name was Joni.Wish I could remember more about it.Clay seems like it would be great for expressing emotion and making something beautiful. There are a ton of great diy(do it yourself) websites geared towards being creative on the web that may offer projects that would be appropriate.Hope this helps and have fun!

If you are using Clay or homemade modeling dough you can roll it out or pat it down with your hand. Use cookie cutters of various shapes to cut out the dough. Use a small straw from a drink box to poke a hole in the top. Let dry or bake. Put ribbon through the hole and tie in a loop so that you can hang your decoration. Paint with felts, sponge and acrylic paint or glue glitter. You could try stamping patterns on paper to make wrapping paper or book markers. Stenciling is also good, cut out shape from mac tac (sticky shelving paper) then peel off covering and press onto fabric. Dab on acrylic paint on the stencil with sponges or brushes depending on ability to hold them. Let dry and then peel mactac off fabric. A perfect way to ensure a nicely stamped figure. Use on blank pillow cases, aprons, or fabric squares for place mats. No ironing, not too much mess.
Collages are also easy and fun plus very nice to display. Using either poster board or plain thick fabric (or burlap). Cut out desired size and use two sided tape to hold onto table. Have white glue on a plate for each artist. Use sponges or paint brushes. Have lots of different things for them to glue on their project, such as bits of wool, small fabric pieces, small pasta, beads, colored paper, textured paper and whatever else you may have that is not heavy and would glue easily. Dab and glue to cover the page. Let dry. Projects can be stapled to strips of wood for display or use thumb tacks if you have a cork board for displaying your work. Another idea is to do tie dying. Using a white or light plain cotton t shirt, so the color shows up clearly, gather some of the material together and tie with string or use elastic bands. Prepare different colors of fabric dye in glass or metal bowls. Dip the fabric you have tied up into a dye using different colors for each bunch or the same color. Let dry and untie the string (remove elastic). You will have a colorful starburst pattern on the Tshirt. One other craft you could try is paper mache. Mix your paste from flour and water and some salt. Cut or tear strips of newspaper, make lots. Blow up balloons to cover or use bowls turned face down. You can cover the balloons without any more preparation but the bowls you will need to put saran wrap or wax paper over the bottom of the bowl first. Dip a strip into paste and place on bowl or balloon. Cover the whole area and then let dry. Try not to make covering too thick or it will take a long time to dry. When your project is dry you can remove the bowl, just leave the balloon in. Use acrylic paint and sponges to color your work and when dry use an acrylic varnish or Gesso to seal and add a shine to it. The bowl is a useful object, candy dish or if it is big a fruit or bun bowl. The balloon you would glue a long piece of ribbon to it and tape a loop made with ribbon or string at the top and hang up for a decoration. There are lots of ideas for adapting craft tools so that someone with disabilities can handle them. The arthritis support group in your area could help you there.
Try to do projects that create something useful or decorative so that your client has a purpose for the activity rather than just play. This is good for their self esteem as well as fun to do.
If you want more ideas try contacting a seniors centre, ask for the recreation or activity co ordinator, he/she should have some good projects for you specially suited to the needs of your clients. Also, the library would be a good resource. Tell the librarian what you are looking for and he/she will be able to help you find the right section to look in.
Hope this helps.
