I’m specifically talking about sites like CBS’s innertube. If I am out of the country (US), which I currently am, and happen to be in a country with horrible TV programming, where I currently am, and I just want to catch an episode of CSI, I get the US viewers only message. What gives, how do they know and can I work around it?

They have the ability to identify the ISP from which you are connecting. If it is out of the country, it's blocked from access. Perhaps a real crack whiz might know how to get around that (by spoofing the IP address to make it look like it's US based) but that is WAY beyond my capability.
