Look at the rollers inside the mouse that the ball rubs against. The rollers is what translates your movements to the mouse cursor on the screen. Use your fingernail or a small, blunt object to clean them. Some mice have spring-loaded rollers, where the spring pushes the roller against the ball. The spring usually wears out after some time. You can open up the mouse and put something behind the spring to help push the roller against the ball.
An optical mouse can be had for $10, so it would be probably better to just buy a new one.

What's so good about optical mice?
Optical mice have no balls!

SPL's answer is good but to add to that if you have cleaned the ball and the rollers and it still is a liitle off, open up the mouse and empty the dirt particles from inside. Sometimes even when you clean dirt gets up inside and interferes with the wheels and you get erratic behavior. If this is not the problem, then I guess new mouse.

It needs a better cleaning then.

If it's optical (has a little light) then you make want to check for debris on the optic sensor (hair, dust, crumbs, etc.)
If it's a ball, remove the ball and scrape all THREE rollers. The vertical, horizontal and diagonal roller (diagonal is the little white one in the corner... no one ever cleans it because it's more difficult).

If all else fails, run to any computer store and get a new mouse.

Even though you cleaned it sometimes they still don't work right. Invest in an optical mouse. You'll be glad you did. PS they're cheap.
