I really can not see much difference, maybe because I am not a native speaker of the language. Helllllllllllllp - :-)

The word REACH can mean (physically) being able to touch something...if a book is on a shelf that is really high, you may not be able to REACH the book...it could also mean a way of contact...if someone never answers their phone, they are hard to REACH...and the person above me is also correct in saying that it means to achieve or attain, as in a goal, desire, or aspiration...

To REACH OUT can be a verb meaning to physically out-stretch your arm to touch something...like the song that says "Reach out and touch somebody's hand".......and it could also mean to make contact with someone, such as trying to talk to them or communicate with them...

There are multiple uses for both terms...

To reach means to achieve your goal or get to where you are going. So if you were on a journey to the next town when you arrived you would have reached your destination.

Reach out means to hold out your hand as, for example, in picking fruit from a tree. You would reach out with your hand to grasp it.
