All three are alpha males, and even when the male is a female (as in the case of a reigning Queen or a female president, or even a female Pharoah, who glued a beard to her chin and everyone politely pretended she was a man) the rules still apply. Usually, when a country drafts its incorporation charter, it decides whether it wants a Prime Minister or a President. Kings and Pharoahs are slightly different in that they are hereditary positions, but both kings and pharoahs have been overthrown and their positions usurped. Emporers are often self-declared leaders, frequently having risen to that rank through a military coup.
If you're asking about the nomenclature, it's usually popular choice when the country draws up its initial Charter. In the case of the United States, they chose president, largely because England had a Prime Minister and they didn't want to copy anything English.

They mean, people holding those titles are all in the same boat - at times a sinking one like the one Blair and Bush are in!
