Much depends on the size of the cabinet, fixtures it is over, etc. Generally, the horizontal center of the cabinet should be at your forehead's height. In a word: place it where it is most convenient for you to use. Good Luck.

There's no such thing. Ceilings aren't a standard height. Medicine cabinets come in all shapes and sizes. Homeowners are not all the same height. Even vanity sink cabinets aren't just one height. You have to look at each situation, and choose a proportionally correct cabinet. Then, check with the owners for what they feel looks right. Typically, I show people cabinets starting about 16" up from the sink rim. Then we tweak it up or down, no more than a few inches, to a height that works for them.

Actually a personal preference. If it has a mirror, do you want to be looking at your belt buckle with the top of your head cut off? Most cabinets are installed over a sink. You want it to look "even". Reach a good compromise with the distance between the sink and the ceiling.

I install at a average of 50 inch from floor, if it has a mirror it will leave the center at eye level with a 5'-2" person, so like one other answer it is 18" from top.
