I have tendinitis and the splint my doctor gave me is not helping the pain in my thumb. I see that sometimes they use a cast for tendinitis. I see an orthapaedic dr. tomorrow. Should I request a cast? I would rather have one because it seems it would stableize my thumb better.

Chances are, if the splint is not working, you will be casted. If not, you can always ask but the decision will be the up to the orthopedic doctor.

Of course, be aware that a cast will bring you a new host of issues to deal with....swelling, much more limited movement, itching (and don't ever stick anything down in the cast!), etc.

Just know that the doctor will do what he feels is best to get you past the tendonitis, to limit the pain and get the fastest recovery possible for you. Good luck!
