Exactly -- first double check that you don't need a visa for the country you're visiting. I've given you a link below that will help you figure out whether or not you do need to apply for one, and how long it will take to get. The amount of time you're going for is also important -- some countries that DO require a visa don't require one if you're there for a certain period of time (which can be as little as 14 days or as many as 90.)
Secondly, check that you have enough validity left on your passport -- many countries require that you have at LEAST six months left from the time you'll be LEAVING the country you visit.
Third, if you're a U.S. Permanent Resident, bring your Green Card with you -- you'll need it to get back into the U.S., if that's where you're living.
Unless you're going somewhere really exotic, it's not too likely you'll need shots or specific medication -- there are also arguments about whether or not malaria medicine is worth the side effects; this is something you'll have to decide if you're going to a country where malaria is prevalent -- but it couldn't hurt to double-check if there's anything else you need. Also make sure you have enough of any medication you're currently taking -- it could be hard to get what you need when you're overseas.
Finally, I know that I personally feel slightly more secure when I photocopy my passport, as well as any credit cards I'm taking, and leave them with a trusted friend or relative. (You can even take a copy with you as well, but keep it VERY SAFE -- the last thing you want is someone shady finding a photocopy of your credit card!) If you should lose or have stolen your passport or cards, having copies of them will make it a whole lot easier to cancel and get new ones as appropriate -- you'll have all the numbers and details to hand, which you'll be especially thankful for in a foreign country.

Depends where you are going, some countries expect a Visa and you can easily check that out on the web. I have traveled extensively in Europe and the UK and these countries only expect a passport. Some of the eastern European countries ask for a visa, notably Russia. The rest of the info you need to have a carefree and inexpensive trip is in the source below.

It depends on where you're going to. You could need a visa or shots for the country you're going to. As far as the US is concerned ... and this IS an international forum, so I'm assuming.... you only need your passport to return.
