Did you have your ultrasound in a hospital imaging department or a free standing imaging center? Then, I know for sure the answer is yes. You must fill out a release of records, but you are entitled to a copy of the imaging report. If this is the case, call the facility where you had your exam and ask how you go about getting a report. Where I work, either you would come in and sign an form, or we can fax you a form, have you return it filled out, then fax or mail you the report.

If you had your u/s done at your doctor's office, I am not sure if they actually write up a formal report. I know that when you have x-rays at a chiropractor or orthopedic office, there is not a separate imaging report. The doctor would say something in your chart that is very short, such as "x-rays were negative".

Of course! All of your medical records are YOUR records. They may charge you a fee for the copy, but you absolutely have the rights to these.
