Suspended license
Car is impounded
Pay for car to get out of impound
You'll need a driver because of the suspended license
Fines
Mandatory Classes (you pay out of pocket for them)
Higher Insurance rates
Now...The worst cases...
You kill a pedestrian(s)
You kill your best friend(s) that's riding with you
You cause an accident that kills uncountable people
The best case...
You kill yourself and nobody else before you cause any of the worst cases.
(Sorry, I may get a thumbs down for this one but there's no sympathy here for someone that CHOOSES to cause these things)

Depends upon state law.
License suspension for 90 days to one year for a first conviction.
Mandatory jail time (1 - 7 days) in some states.
$500 to $5,000 fine, more if personal injury is involved.
Massive increase in auto insurance rates for 3 - 5 years. This can exceed $2,000 per year and is often the greatest hidden cost of a DUI. Mandatory SR-22 filing requirements for 3 - 5 years depending upon state law and sometimes at the judge's discretion.
Mandatory alcohol abuse treatment program in many states, costs paid by you of course.
Possible alco-lock requirement in some states, paid for by you.

Depends on circumstance ... and your attorney.
Usually you lose your license for 6 months - 1 year.
But, the better your attorney - the lighter the sentence.
