I just returned from Chicago consulate doing same thing as you . This is what I took with me . I think you will have to go in there in person . Also in case you do not have the right info. they can tell you right then. They do not answer the phone here in Chicago so good luck on that {smile} . Here is what I needed after two trips in there: Copy of birth certificate, original copy of marriage license and 3 copies , 1 picture of your husband , 1 separate picture of you , 2 new passport pictures , Copy of your passport pages 1-5, copy of drivers license or any valid ID that proves you live here . Also have cash or money order for $75 dollars . If your husband was married before have the divorce decree number on that document . Bring old passport along to . Good luck and write me if you have any other questions or my answer is unclear.

Go to the Philippine embassy in Florida. You need your old passport, marriage contract (if you want to use your husband family name) and 3 copies of passport size picture on white background & collared shirt.

File an application form at the Philippine embassy together with your marriage certificate since you were already married. The fees are just minimal.
