Small lap dogs are allowed in the cabin on some airlines. For example a woman had a small dog on a recent Alaska Airlines flight from Las Vegas to Seattle. Larger dogs must be kept in a kennel in the cargo hold. My dogs have gone through this many times over the past thirty years or so. The cargo hold is pressurized, like the main cabin, but there is no specific area reserved for animals. They go aboard witht he suitcases and cargo. I would talk to your vet about medication for your dog if they happen to be nervous or skittish. Tell them exactly what you are planning to do and how long the expected flight will be. Whatever medication you might give your dog, try it out ahead of time at home so you know what to expect.

I have a close friend who has had to transport several animals on planes and I know that when able, she brings them with her as carry-on. You should check with your airline and see if that is a possibility. If not, I would make the decision based on the personality of your dog. That's a long trip and if he's the anxious type, maybe a little calm down tablet from the vet would be helpful for him. I know you said you are already planning to seek the advice of your vet, that truly is the best way to go. Also make sure you talk to the airline and your vet about all required vaccinations.

I flew my dogs from California to Hawaii 4 years ago. They seemed fine yet my boy dog (Labrador) was soooo shaken by the time I picked him up, he has since freaked out every time we are near airline fumes!!!
To help you, I do know that a dog should not be given any "drugs" to calm him due to the fact that no one will be able to assist if the drugs have a negative effect. That's about all I know. Good, good luck my fellow dog owner :)

well most airlines won't let them travel in the cabin, and they'll go in the cargo hold. However, i believe some asian airlines, or Korean Airlines, does allow you to keep a small dog in the cabin.

They can run away. Two years agao, a famous Show Dog, that was in NYC for the Wesminster dog show, ran away at JFK airport and has not been found.
