i have a shih tzu too. its fine to leave it on but make sure two of your fingers can fit between the neck and collar that way you know its not too tight.

yes its perfectly ok!...but make sure that you check that two of your fingers can fit because puppies are constantly growing and probably will go through 3 collars!( well mine did!) Its also beneficial because if the puppy gets out the tags from rabies shots (hope you got them already because you should have) and also tags that you may have created with your address and phone number will be very helpful if somebody may find the puppy!

Sure. It's the best time to get him used to a collar. Just check it at least weekly, so that it doesn't get too tight as he grows.

a regular collar thats not too tight is fine, however, a choke chain collar is NOT ok for the dog to wear when it is not being supervised, they can get stuck on something (even in a crate) and choke to death

Yes if it is a regukar collar. All of my dogs wear theirs all of the time. Just in case god forbid they ever got loose and someone found them they would have their tags on.
