I don't think Word has a template for this - but Publisher does. Actually, it isn't so difficult. I made one this way:

Table, Insert
8 columns
2 rows
- right-click on the toolbar and choose the Tables & Borders toolbar.
- select the top row. Click the "merge cells" button on the T&B toolbar. Cells merge into one large cell.
- in the top row, type the ad. With the top row selected, click Table, row height. Set the height for the top row to "exactly" 5." (that's 5 decimel point quotes for inches)

- You should have 8 columns below that. Select them. Click Table, cell height, set the row height for exactly 3 or 4."
- In the first cell, type the contact information. It will wrap.
- Select the 8 cells. Click Format, Text Direction. Click on the appropriate direction.
- Now, all you have to do is select the information in the first cell and copy it to the remaining 7.

Put it in a text box, then click Format, Text Direction. Then copy it across the page.
