Typically when you reach failure. However, for weight lifting to be effective, this should be within a certain range, such as 8-10 reps. So, you should do, for instance, 10 reps to failure, meaning you can barely crank out 10 reps and won't be able to do the 11th one by yourself, or need assistance on the 9th and 10th one. That's usually how it should work.

Of course, there are many routine out there that call for more or fewer reps/sets, but for most people, 8-10 reps to failure, 3 sets per exercise is sufficient to stimulate muscle growth.

For each workout, you should be able to either beat the number of reps you did last workout, or go heavier in weight. If you're able to do 10 reps for 3 sets of an exercise without assistance, it's time to go up in weight a bit. If you are stalling out over more than a couple of workouts, then it's time to change to a different exercise. That way, you avoid plateauing.

this is the purpose of the training log.

regardless if the goal is to increase strength, endurance or fat loss you want to perform more work each training session in the same amount of time. say for example your working sets for bench press are 150 lbs x 10 reps today. the next time you bench you either want to use a higher load of perform more reps. so that would be something like 155 lbs x 6-8 reps or 150 lbs x 11-12 reps. the same applies for cardio. if you do 45 minutes on the treadmill and run 6 miles today than the next cardio session you want to run 6.1 miles in the same 45 minutes, etc.
