My grandfather is 90 years old, and very healthy for a person of his age. But his kidneys are performing poorly and losing their functionality everyday. The creatinine level is 6.8 and has been increasing lately.

He has to walk periodically in order to get some exercise, and the doctors say so too. But I think this is for general cases where the kidney function is not so poor.

I need to know if there are any special exercises or methods to keep the kidney healthy, or not make it worse at the least.

Dialysis is the nest step, but are there exercises or restrictions to exercises that should be considered (apart from just walking)?

Considering your grandfather's kidney function is only 15-20%, the kidneys will eventually pack up. It will stop functioning in the end. He is 90 years old and most of his organs may be at the point of packing up. Being active, like walking or doing household work (those he can manage only) can help him. But this does not mean that the kidneys are going to function properly.

There are measures to mitigate or slow the progress of renal failure. Adherence to fluid restriction is very sensible. Looking at his kidney function, he can have about 800-1000 ml of fluid daily (this includes tea, juice, water). Keeping to this restriction will help him prevent pulmonary edema(fluid in the lungs) or swelling on his legs. I always remind my patients that they cannot drink a lot of fluids that their kidneys will not be able to excrete.

He also have to avoid foods that are high in potassium, salt and phosphate. The kidney cannot excrete the potassium and phosphate thus this will build up in the body. Your grandfather also has to take those medications that are prescribed to him. Those medications are not prescribed without any reason at all.

Most patients with renal failure feel always tired and nauseated. This is part of the disease process. Encourage your grandfather to be active as he can and have a proper diet. Eat foods that he is allowed and avoid those he cannot have. Take your grandfather out for a walk to the park or the shops for this can help him.

At some point your grandfather will be on dialysis. There are a lot of people his age (where I work) is on dialysis. You can expect this to people his age. I hope as early as now the renal medical team or nephrologist has already discuss about his options when dialysis is imminent. It is actually advisable that you know what your options are so that you can be prepared on what will happen in the future.

There aren't any specific exercises for kidneys, but physical activity is recommended.

I hate to sound like a naysayer here, but he's 90 years old. If he can walk a half mile a day he's doing great. I wouldn't try to get a 90 year old man to do any strenuous exercise even if one did exist.

Dialysis is the next step, but he may not get to that step. Kidney failure can just stop progressing sometimes, so it may just stop getting worse, and he can go on without the dialysis. If he does end up on the machine, the doctor may give him some activity restrictions, but it's likely that he can continue the walking whenever he feels up to it.

the walking is to optimize the total efficiency of his body, that helps the kidneys function, but it does nothing to improve the kidney's ability to function.Eventually the kidneys will fail, and he will need dialysis, at 90 years old his body is wearing out, something we all face eventually.

Poor Guy, he may need dialysis. i feel for him i been there. And for older people it is hard for them. keep him happy and let him walk as he wants. I will pray for him.
