I took the CPR course through a local college in Kansas in the fall semester (2006).

How/who do I request a replacement CPR card from?

Most CPR Certificates are valid for three years.

With that being said, you must know the granting agency American Red Cross, Green Cross, Safety Council, etc. Call the College and ask them.

Then simply call the granting agency and ask them how to obtain a replacement card.

The replacement card would come from the issuing agency- Red Cross, Heart Association, etc.

The person who taught the class might be able to help as well- either with the card or some proof of enrollment.

Functionally, getting a new card is tough- they just are not geared up to do that a lot of the time. Most of the reaons you need it for can usually be handled with a copy of the class roster, etc. until it is time to renew- then you get a new card and all is OK.

If its already expired you have to take the class again in order to get it renewed. If you just lost it then you can probably call the AMA and tell them you lost yours and they can send you a new one. But you have to take the class every year usually because the rules and stuff change often.

Call your local health department. I believe the certifications come through them.
