My shoulders often ache from sitting at a PC all day, and I too can feel knots in them. I find that weightlifting can help- it exercises the knotted muscles and loosens them up. Swimming is also pretty good for it.

I hope you find something to help, I know it can be a killer!

There is an exercise called the "turtle" that always works for me. Sit up right and make sure you do not move your neck - just your head. Move your chin forward slightly like a turtle would do - then draw it back again like a turtle going in to its shell. It is a very slight movement - but one that has big impact on the shoulder and neck muscles. The key is to limit the movement to your chin/head and don't let your shoulders or neck move. It's worth a try.

A doctor of chiropractic medicine can break down the knots and help you feel much better. I've enclosed to link where you can find a chiropractor in your area.

acupuncture is also very good for this condition. You might want to see an acupuncture doctor.

What's wrong with using heat? A hot shower will help!

Apart from that, take a couple ibuprofin.

Look for a Chinese muscle rub in a bottle that's called, "Red Flower". My wife says that old injury like yours should be treated with cold and ibuprofen. Good luck.

Exercise of the arms and shoulders. With light weights. Try it and you will see how good it is.
