gout is a chronic type of inflammatory arthritis that causes immediate attacks of pain, tenderness, redness, warmth and swelling in some joints. Usually, the joints in the big toe are affected, but gout can also affect the instep, ankles, heels, knees, wrists, fingers and elbows.

Gout is caused when there is over production of uric acid in normal purine metabolism in the body.

Gout is an arthritic condition involving uric acid crystal deposits in the joints. The most common joint involved are the big toe joints but it can happen in any joint in the body. It is extremely painful and gets worse with drinking alcohol, eating rich foods, and taking diuretics. If you are having a gouty attack, drink lots of water and seek a doctor's evaluation to be placed on medications to decrease the inflammation and prevent further attacks if possible.

Gout is precipitation of monosodium urate crystals into tissue, usually in and around joints, most often causing recurrent acute or chronic arthritis. Acute arthritis is initially monarticular and often involves the first metatarsophalangeal joint; symptoms include acute pain, tenderness, warmth, redness, and swelling. Diagnosis requires identification of crystals in synovial fluid. Treatment of acute attacks is with anti-inflammatory drugs. Attacks are prevented with NSAIDs and/or colchicine, and, for persistent hyperuricemia, allopurinol or uricosuric drugs.

Excess uric acid in the body is recognized as the cause of gout. The excess can be caused by:


increase in production of uric acid by the body.
by under-elimination of uric acid by the kidneys.
by increased intake of foods which contain purines which are metabolized to uric acid in the body.

Gout occurs when there is a buildup of urate, which is formed from the breakdown of purines. Purines are nucleic acids and are important in many bodily functions, especially for building DNA. (You may recognize adenine and guanine as building blocks.) Problems with disposal of urate happen when there is too much or too little of an enzyme: for example too little HGPRT as in Lesch-Nyhan syndrome or too much PRPP. (I won't bore you with the full names of the enzymes- you can look them up if you want.)
