The chairman would normally say, "I declare this meeting (or conference, if the whole conference is over) adjourned."

"This concludes the conference" sounds like either a report or an introduction to the final event at the conference. Another possibility would be "The conference is now concluded" but it sounds less like the chairman is in control.

No, it isn't. It can only be used at the end of a conference and it wouldn't normally be the chairman who would say this, it would be the MC of the event.
