My dogs are trained to go outside every time they come out of their crates, before anything else. In the mornings, I let them out, they go straight outside. After they come in, they eat.
It is the same routine when I get home from work. Anytime I leave the house, they stay in the crates. When I get home, outside. They will eventually fall into the routine.

At 3 months he only has limited bladder control. Keep him in the room you're in so you can watch him. Take him out after a nap, after hard playing, a long drink, if he's restless (head down, circling like he's looking for somewhere to go), or just hasn't been out for a while.
You need to go out with him, tell him "go pee", give him lots and lots of praise the second he does. then come right back in. If you don't go out with him, tell him what to do and then praise him for doing it, he has no way of knowing when he's doing it right. Dogs learn really quickly this way, with lots of positive reinforcement.
If he has an accident, quietly clean it up. If you catch him in the act, tell him "no!!", take him out, and praise if he finishes outside.

What you do is if it goes to the bathroom in the house, punish it and take it outside right away! Then say "go potty, go potty, go potty." If it goes, give it a treat and say "good potty!" (Because if you say good boy/girl, they have no idea what boy/girl means so there is no point in saying that. Try taking it out every hour and say "go potty, go potty go potty." Make sure you do it in a nice voice though. Then if it goes potty OUTSIDE, say good potty and give it a treat!
