find calorie/fitness calculator on the net, enter all your stats and it will tell you how many calories you need a day to maintain, take off 300-400cal off that number and thats how much you need to lose.
eat 5-6 small meals a day (eat every 2-3 hours)
5-8 servings of fruit and veggies a day
8 glasses of water
have complex carbs for breakfast - they give you energy
have lean meat (protein) for dinner - repairs muscle
cardio exercise 4-6 times a week for 30-50min, light weight training
dont consume foods that are made of white flour (white bread, cakes, past etc.), sugar loaded foods (cookies, icecream, candy etc) and nothing fried, oily.
ofcourse you can spoil yourself once in a while with a little treat:)

It sounds like you need a trainer! In order to tone you will lose weight in the form of body fat. This is the only way to uncover the muscle underneath the fat. There are no cookie cutter programs. Your body and your activity level must dictate how many calories you take it, what foods you eat, and what exercises you do. First find out how many calories to take in on a daily basis. Then begin to put together a training program that includes cardio, weight training, and stretching.

Go vegan/vegetarian, in addition to saving animals, it works for most people trying to lose weight. And when done properly it is a high protein, low fat diet, so i would recommend it.

That's aggressive for 3 weeks, but you can do it if you are serious.

Eat nothing but raw vegetables (salads).
Drink only water.
Run for 30 minutes every day.
