Yes you can still file, and if you had income above certain limits depending on your filing status, you might be required to. If you have a refund coming, you'll still get it.

You use your current address, since that's where any notifications, and your refund, will be sent. List your main job as it was then.

You need to use your current address and all of the financial information for 2004, including any W2's issued by employers for that year. It doesn't particularly matter what you list for occupation, but I would put your 2004 occupation on the appropriate line of the 2004 tax return.

Use your current address. Report the income from 2004 from whatever source(s) it came from.

The occuption should list whatever your primary occupation was in 2004. That CAN have an impact on how the IRS evaluates your return for audit purposes.
